Below please find a list of required documents for the city you are applying for.

Background/driving record checks are done at the end of the on-boarding process and deducted from first payment.


NEW YORK CITY

1. Driver’s License
2. Social Security card or proof of EIN if incorporated
3. TLC (Hack) License
4. TLC Permit or Inspection Report (we need to know your Base name & number)
5. Vehicle Registration
6. Insurance Certificate ($300K minimum, GroundLink needs to be added to your insurance)
7. Certificate of Liability
8. Copy of voided check (with your name) or a Direct Deposit form
9. Photo (face only) for your profile


NEW JERSEY

1. Driver’s License
2. Proof of EIN
3. NJ MVC Certificate
4. Vehicle Registration (confirm “OL” plates)
5. Insurance Certificate ($1M minimum, GroundLink needs to be added to your insurance)
6. Copy of voided check (with your name) or a Direct Deposit form
7. Photo (face only) for your profile


CONNECTICUT

1. Driver’s License
2. Proof of EIN (company should be registered in CT)
3. Public Service License (PSL)
4. General Livery Service Permit
5. Vehicle Registration
6. Insurance Certificate ($1M minimum, GroundLink needs to be added to your insurance)
7. Certificate of Liability
8. Copy of voided check (with your company name) or a Direct Deposit form
9. Photo (face only) for your profile


LONG ISLAND (NASSAU COUNTY)

1. Driver’s License
2. Proof of EIN (company should be registered in Nassau County)
3. Nassau County TLC (Hack) License
4. Vehicle Registration
5. Insurance Certificate ($1M minimum, GroundLink needs to be added to your insurance)
6. Certificate of Liability
7. Copy of voided check (with your company name) or a Direct Deposit form
8. Photo (face only) for your profile


LONG ISLAND (SUFFOLK COUNTY)

1. Driver’s License
2. Proof of EIN (company should be registered in Suffolk County)
3. Suffolk County TLC (Hack) License
4. Vehicle Registration
5. Insurance Certificate ($1M minimum, GroundLink needs to be added to your insurance)
6. Certificate of Liability
7. Copy of voided check (with your company name) or a Direct Deposit form
8. Photo (face only) for your profile


CHICAGO

1. Driver’s License
2. Proof of EIN (Employer Identification Number) – typically a letter from the IRS (all drivers have to be corporations)
3. Public vehicle license
4. Livery or Public Chauffer license
5. Photo (face only) for your profile
6. Vehicle Registration
7. Insurance Certificate ($1M minimum, GroundLink needs to be added to your insurance)
8. Certificate of Liability
9. Copy of voided check (with your name) or a Direct Deposit form


BOSTON

1. Driver’s license
2. Proof of EIN (Employer Identification Number) – typically a letter from the IRS (all drivers have to be corporations)
3. Copy of voided check or a completed Direct Deposit Form
4. MassPort Ground Transportation Permit (sticker, number must match livery plates)
5. Photo (face only) for driver profile
6. Vehicle registration
7. Certificate of Liability (GroundLink needs to be added to your insurance, $1M minimum)


ATLANTA

1. Driver’s License
2. Proof of EIN (Employer Identification Number) – typically a letter from the IRS (all drivers have to be corporations)
3. Photo (face only) for your profile
4. Vehicle Registration (“LM” plates)
5. Ground Transportation Revocable Permit from Hartsfield-Jackson to be able to pick up and drop off at the Atlanta airport
6. Insurance Certificate ($1M minimum, GroundLink needs to be added to your insurance)
7. Certificate of Liability
8. Copy of voided check (with company name) or a Direct Deposit form


HOUSTON

1. Driver’s license
2. Proof of EIN (Employer Identification Number) – typically a letter from the IRS (all drivers have to be corporations)
3. Copy of voided check or a completed Direct Deposit Form
4. Vehicle for hire driver’s license (issued by City of Houston)
5. Vehicle permit
6. Port of Galveston Facilities Access Permit
7. Houston Airport Systems (HAS) permit
8. Photo (face only) for driver profile
9. Vehicle registration
10. Certificate of Insurance ($1M minimum, GroundLink needs to be added to your insurance)


DALLAS

1. Driver’s license
2. Proof of EIN (Employer Identification Number) – typically a letter from the IRS (all Dallas drivers have to be corporations)
3. Copy of voided check or a completed Direct Deposit Form
4. Copy of driver permit (issued by City of Dallas)
5. Vehicle permit (a sticker on vehicle windshield)
6. Copy of DFW airport permit
7. Photo (face only) for driver profile
8. Vehicle registration
9. Certificate of Insurance ($1M minimum, GroundLink needs to be added to your insurance)
